Sir Charles Gairdner Hospital Specialised Falls Service – Nedlands

The Specialised Falls Service at Sir Charles Gairdner Hospital (SCGH) provides a comprehensive risk assessment and management service to evaluate older adults at risk of falls. This can occur either through a Home Visit or a Clinic based assessment.

The Home Visit assessment is performed by a Physiotherapist or Occupational Therapist who visits the client at home and performs a comprehensive assessment of risk factors that can contribute to risk of falls. This includes assessment of balance and walking difficulties, home hazards and medication. The therapist then implements a management plan to reduce a persons risk of falls which is done in conjunction with the client.

Clinic based services are performed on site at SCGH and consist of comprehensive assessment by Medical Specialist, Physiotherapist and Occupational Therapist. Management plans are implemented after assessment in consultation with the client.

Treatment options are also provided which includes specific balance exercises delivered as home or group exercise programs, Tai Chi, tailored falls risk education, home equipment and installations, specialist medical advice, medication review and further investigations of medical issues identified that may be contributing to risk.
