Meet the Improve Your Health grant recipients

With the next Stay On Your Feet® Improve Your Health campaign launching next week, Injury Matters congratulates the successful Improve Your Health grant recipients who are ready to commence their community-based grant programs.

Improve Your Health is one part of the Move Improve Remove messaging that encourages older adults to stay active, independent, and prevent falls. Improve Your Health looks at simple changes which can improve a person’s health and wellbeing, focusing on nutrition, cognition, and medication management.

Meet our successful Improve Your Health grant recipients for 2023.

City of Bunbury

This eight-week program, implemented by the City of Bunbury at the South West Sports Centre, will support the pilot of a new program for older adults in the City of Bunbury area. The program will be designed with low-impact principles to increase participant strength and fitness, with easy-to-follow movements and exercises to improve range of movement, posture, and balance. The program will see two 60-minute group sessions led by a personal trainer each week, with each session consisting of a:

  • 30-minute physical activity session
  • 30-minute educational talk covering different topics based on the Improve Your Health campaign messaging

The program will assist in bridging the gap for older adult community members with a lower baseline of strength and balance.

City of Mandurah

The City of Mandurah aims to increase knowledge and awareness among the older adult population in the Mandurah area around Improve Your Health falls prevention strategies. The City of Mandurah will deliver the following 90-minute workshop sessions over three weeks, focusing on different falls prevention topics for older adults:

Workshop 1: Fuel Your Body with a nutritionist from Foodbank

Workshop 2: Check Your Medicines with a local Pharmacist

Workshop 3: Keep A Healthy Mind art class to stimulate the brain

Pramana Medical Group

Pramana Medical Group will deliver an Improve Your Health program that aims to reduce the risk of medication-related falls among their existing older adult clients. The inclusion criteria for the program will consist of:

  • Currently taking five or more medications
  • A recent discharge from hospital
  • Uncontrolled chronic health conditions

Participants will be screened by the Pramana Medical Group Consultant Pharmacist for a range of falls-related risk factors and be referred to their GP or allied health support at the Medical Centre for falls prevention intervention.
